On the Front of the Brochure:

  1. List the department(s)/institution(s) presenting the conference.
  2. List the title of the conference.
  3. List the date and time of the conference.
  4. List the conference location.
  5. Tell the reader why they should attend the conference.
  6. Include the appropriate CME Sponsorship statement:

For Directly Sponsored Programs:
“Sponsored by the Office of Continuing Medical Education of New York Medical College.”

For Jointly Sponsored Programs:
“Jointly sponsored by the Office of Continuing Medical Education of New York Medical College and {insert name of joint sponsor here}.”
 

Inside the Brochure:

  1. State the target audience.
  2. List the program agenda, or at the very least, list the topics to be covered at the conference.
  3. Provide directions to the conference location.
  4. List the Program Chairman.
  5. List the Program Faculty – be sure to include degrees, positions, and affiliations.
  6. List the conference learning objectives. Objectives should be specific and measurable. (Refer to the action verbs list for help in writing learning objectives.)
  7. The registration form:

Note: Be sure the registration form is not printed on the back of the directions to the conference location.

A. Include the conference title

B. Include the conference date

C. Include the conference time period

D. Include the conference location

E. Include ADEQUATE space for registrants to fill in the requested information.

Suggestions include:

• Name
• Degree
• Business Address (allow 2 lines)
• Business Telephone
• Business Fax
• Email Address
• Specialty
• Organization/Business/Hospital

F. State Conference Costs

G. State how conference costs may be paid – include to whom checks should be payable and an address to mail checks. If credit card payments will be accepted specify what cards will be honored. Be sure to include a space for the card number, expiration date, amount to be charged to the card, and a signature line.
If you will assess a cancellation fee it MUST be disclosed here.
Include any registration cut-off date here too.

H. Include a name and number to contact for further information regarding the conference. Be sure this number will lead potential registrants to a person who can answer questions and provide directions.

  1. Accreditation and Disclosure:

A. Include the appropriate NYMC Accreditation statement:
Note: Ensure that the number of credit hours requested has been approved by the NYMC Office of CME before placing it on the brochure.

For Directly Sponsored Programs:
“New York Medical College is accredited by the ACCME to provide continuing medical education for physicians.
New York Medical College designates this educational activity for a maximum of {enter # of credits here} AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.”

For Jointly Sponsored Programs:
“This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of New York Medical College and {Enter name of Joint Sponsor}. New York Medical College is accredited by the ACCME to provide continuing medical education for physicians.
New York Medical College designates this educational activity for a maximum of {enter # of credits here} AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.”

B. Include the NYMC Disclosure Statement:

“All activity faculty and planners participating in continuing medical education activities sponsored by New York Medical College are expected to disclose to the audience any significant support or substantial relationship(s) with providers of commercial products and/or devices discussed in their presentation, and/or with any commercial supporters of the activity. In addition, all faculty are expected to openly disclose any off-label, experimental, or investigational use of drugs or devices discussed in their presentation.”

  1. Include an acknowledgement of commercial sponsors and other supporters of the conference.
    Note: This acknowledgement can not reference any specific product(s).
  2. Include the NYMC logo.
  3. Be sure to include return address for the mailing portion of the brochure.
  4. Include the conference title, date, and location on the mailing portion of the brochure.
