Are you receiving a diploma or certificate? Please read the important graduation information below


Apply for Diploma:

Applications for the May 2015 Graduation are due by December 1, 2014.

May is the only time of year that official diplomas are awarded and an official ceremony is held.  The 2015 graduation ceremony will be held at Carnegie Hall on Thursday May 21, 2015 at 5 p.m.  If you would like to apply, please do the following:

  1. Complete the Graduation Application and return it to the Registrar’s Office by December 1, 2014.
  2. Submit required fee with application (GSBMS $120) (SHSP $140) (MD, DPT, MS Speech $0). If applicable, we accept check, money order or credit card.
  3. Submit a new form even if you have previously applied (no fee required).
  4. MD/MPH & MD/PHD only need to submit one form. However, fees for the MPH $140 or PhD $120 are required with the application.
  5. Get approval from your Program Director (GSBMS students only).
  6. Submit required NORC survey with application (Available in the Registrar’s Office for PHD only).


Special Note:

Diplomas are given out on stage at Carnegie Hall.  Therefore, if you indicated on your application that you would be attending commencement and later become unable to, please notify us in writing.  Please include your name, school, signature, and instructions to either pick up your diploma or have it mailed.  Diplomas are not mailed overseas or to P.O. boxes.

Apply for Certificate:

Certificates are offered through the School of Health Sciences & Practice.  Certificates are awarded the term following the completion of all course requirements as follows:

To apply for a certificate, please complete the Certificate Issuance Request Form and return it to the Office of Admissions once you have completed all the required course work (no fee required).
