Human Resources - 2010 Open Enrollment

The 2010 Open Enrollment for the College's Medical, Dental, Vision, and Flexible Spending Account Plans is currently in progress. The 2010 Open Enrollment deadline for the medical, dental, and vision plans and the Flexible Spending Accounts has been extended to Wednesday, December 30, 2009.

We have continued the online Open Enrollment process this year to make it more efficient and convenient for you to review all of the Open Enrollment material from your office or personal computer. Instead of receiving an enrollment packet, you can review the 2010 Open Enrollment material by navigating through this website.

You can print any form required to make a change or enroll in the medical, dental, flexible spending account or vision plans. Simply scroll down to the end of this letter and select the items you wish to view or print.

If you are currently enrolled in the Empire/Care Plus or Oxford medical plans or one of the MetLife dental plans, and you do not wish to make any changes in 2009, no enrollment forms are required!

Please note that enrollment forms are required if you decide to enroll in the Flexible Spending Accounts and/or the Vision Plan.

WHAT’S NEW IN 2010?

Care Plus PPO Plan

Below is the Care Plus PPO Plan Summary, highlighting changes that become effective on January 1, 2010.

Please note increases to your deductibles, copays, out-of pocket maximums, and specialist office visits.

In addition, the $20 copay for most preventive care benefits will be eliminated. Those benefits will be paid at 100% in-network effective January 1, 2010.

New Empire ID cards will be mailed to your home in December.

CVS/Caremark Maintenance Choice Program

Employees who are taking a long-term medication can choose to receive a 90-day supply by mail through CVS/Caremark Mail Service Pharmacy or to pick up your prescription at a local CVS pharmacy. Whether you choose delivery or pickup, you will pay the same copay.

CVS/Caremark will mail additional information about this program to your home in December.

AETNA HMO

Due to low enrollment and higher premiums, the Aetna HMO will be terminated effective January 1, 2010. Employees who are enrolled in this plan can elect the Oxford HMO or the Empire/Care Plus PPO Plan.

DENTAL ASSISTANCE PLAN

There will be a biweekly premium for the Dental Assistance Plan effective January 1, 2010. Please see the 2010 rate chart below for details.

1. Print and complete the online Master Enrollment Form.

2. Print and complete the online Empire or MetLife Dental Plan Enrollment Form.

3. Return all forms to NYMC, Human Resources Department, Administration Building, Valhalla, NY 10595.  Attention: Open Enrollment 2010.

Please note:  Deadline for all enrollments/changes is December 30, 2009.  All changes become effective on January 1, 2009.

CarePlus PPO Plan participants will receive new ID cards from Empire and Caremark Pharmacy for use on or after January 1, 2010.

If you are currently enrolled in one of the medical and/or dental plans, and you do not wish to make any changes in 2010, no action is required!! However, you must re-enroll each year in the Flexible Spending Account Plans or the Vision Plan.

If you change medical plans, several weeks after enrollment you will receive an ID card for the plan you choose. No ID card is required for the dental plans. Your new coverage becomes effective on January 1, 2010.
