The College form entitled "Application to Conduct Sponsored Program or Do Research" must be completed for all applications for sponsored programs and for all research projects whether or not they are extramurally funded. Select the appropriate form to download and open a template for completion on screen. Save to your hard drive or disc and print.

A checklist summarizing the items required for each type of application is provided. The checklist is intended as a guide, it is not necessary to submit it to the Office of Research Administration.
