Preparing for VSAS/Electives

Health Services Portion 

NYMC Health Services completes the health requirements forms.
Please follow these instructions:

Before You Deliver Elective Forms to Health Services:

  • READ all Elective Immunization/Health Forms:   All institutions have different requirements.
  • Schedule an annual physical exam with your primary care provider within one year of your elective dates.  This is required by some electives.  Make sure you receive a copy of your physical with the date and the physician’s signature because Health Services will need this to complete your forms.
  • Clearly print your name and the required demographics at the top of each form.
  • DO NOT obtain additional titers or PPDs before your forms are reviewed by Health Services. This can result in problematic situations that delay form completion.  For example, titers documenting immunity at matriculation are sufficient and a repeat titer may become negative over time and is not needed.
  • Prioritize your electives.  Health Services completes the first 5 per student, and then additional electives after your classmates’ 5 forms are completed.

Bringing Your Forms to Health Services:

  • Hand deliver your forms or FAX the forms to Health Services 914-594-4692. Please do not e-mail forms.  If applicable, include a copy of your physical. If greater than 5 forms: Label in priority order #1= highest priority.
  • If you choose to have your elective forms completed by your physician’s office, they must be completed in its entirety and signed by the physician.

What to Expect from Health Services: 

  • Our nurse practitioner will complete the forms and you will be notified via NYMC email as to any needed tests or vaccines.
  • When your forms are complete, you will be notified by Health Services to pick up the forms or the forms can be batch scanned and emailed to you.
