Request a Faculty Appointment, Promotion and/or Tenure

Department Chairs make recommendations for faculty actions to the Dean of the School of Medicine.  To assist chairs and faculty in this process the following resources are provided:

Applications are considered on a rolling basis and are complete when the Office of Faculty Affairs has received the following:

• Letter of Recommendation from the Department Chair addressed to the Dean of the School of Medicine
• Chair Request for Faculty Action Form
• Faculty Personal Information Form
• Professional References Form
• CV in NYMC SOM Format
• Examples of Scholarship that best represent your academic activity; provide three examples (e.g., peer reviewed publication, etc.); required only for associate professor and professor ranks.
