Certificate-Seeking Students

Apply for Certificate:

Certificates are offered through the School of Health Sciences & Practice.  Certificates are awarded the term following the completion of all course requirements as follows:

Once you have completed all required coursework, please complete the Certificate Issuance Request Form and return it to the Office of Admissions in order to be awarded your certificate (no fee required).

Please note: M.P.H. students who wish to add a certificate to their academic program should submit an Application for Certificate to the Admissions Office at least one semester before graduation.
