This is to be used as a referral form designed for faculty, staff, and students to communicate non-urgent, non-emergency student mental health concern(s) to the Office of Student Mental Health & Wellness. This can be anonymous, or you can provide your information.
If you wish to refer a student or have a student of concern that you would like to discuss with one of our staff, please proceed by completing this form. It will be sent immediately to one of the clinicians who will respond accordingly. Please be aware you can always call the office during normal business hours at (914) 594-2400 as well.
